29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: On or about 02/19/2019, North Central Stamping located at 920 Lone Star Drive, OFallon, MO, had a forklift which was modified without the manufacturers approval. The original horn switch was no longer functional so a separate horn switch/button was mounted on the left side of the steering column without the manufacturers approval.

29 CFR 1910.1200(g)(8): The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical: On or about 02/19/2019, North Central Stamping located at 920 Lone Star Drive, OFallon, MO, did not update their Hazard Communication Program with the required Safety Data Sheet (SDS). There were products, such as, Anchorlube G-771, 3EE2-Lectra Clean (R) Degreaser, and 3139 Consultant Lubricants, at this facility without SDS.
