29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On and before January 19, 2017, in the Injection Molding Area of the facility, the employer had not developed and implemented Machine Specific Energy Control Procedures for Kantus - H2200 Injection Molding Machine. As a result, employees are exposed to serious injuries in the event of an un-expected start-up or movement due to not properly identifying and locking out potential energy sources.

29 CFR 1910.147(c)(6)(i)(B): The periodic inspection of the energy control procedure conducted did not correct any deviations or inadequacies identified: (a) On and before January 19, 2017, during a prior Audit Inspection for the Kantus - H2200 Injection Molding Machine, the employer did not recognize and correct the apparent inadequacy of not having Machine Specific Energy Control Procedures developed and implemented. As a result, employees are exposed to serious injuries in the event of an un-expected start-up or movement due to not having properly identifying and locking out potential energy sources.
