29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program was understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls were acquired by employees. Instance A: Maumee Assembly & Stamping, LLC located in Maumee, Ohio: On or about May 11, 2017, the employer did not ensure employees were trained on the safe application, usage, and removal of lockout devices. Employees did not lock out blanker #1 when performing coil steel feeding activities inside of the press. Instance B: Maumee Assembly & Stamping, LLC located in Maumee, Ohio: On or about March 24, 2017, the employer did not ensure employees were trained on the safe application, usage, and removal of lockout devices. Employees did not lock out the shaker table on Line #5 press #4 prior to performing cleaning activities.

29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the following elements and actions and were not done in the following sequence. a. Maumee Assembly & Stamping, LLC located in Maumee, Ohio: On or about May 11, 2017, the employer did not ensure employees responsible for performing the coil change out operation on blanker press #1, serial # 24680, model # S4-1500-186-108T, locked out the equipment prior to exposing themselves to pinch points created by the potential unexpected startup of the equipment when standing on the bolster plate to manually lift the leading edge of the sheet steel to place it into the die. b. Maumee Assembly & Stamping, LLC located in Maumee, Ohio: On or about March 24, 2017, the employer did not ensure the shaker table on line #5, press #4 was isolated from its hazardous energy source prior to an employee removing scrap, exposing the employee to crushing hazards between the shaker table and the press's die. The employee sustained a severe crushing injury to his thumb when the shaker unexpectedly started and crushed the employees thumb between the shaker table and the die.
