29 CFR 1910.217(d)(7): Provision was not made in both upper and lower shoe(s) for securely mounting the die to the bolster or slide of mechanical power press(es): a) On or about July 31, 2017 the employer did not ensure the die/adapter plate in press #50-1 was securely mounted to the ram. Employees were exposed to crushing hazards when the top die fell striking the bottom die during operation.
