29 CFR 1910.217(e)(1)(i): A program of periodic and regular inspections of mechanical power press(es) was not established and followed to ensure that all parts, auxiliary equipment and safeguards were in a safe operating condition and adjustment: a.) On or about September 21, 2017, the employer did not establish and follow a program of periodic or regular inspections for the Verson BLK2 mechanical power press to ensure that all parts, including the clutch/brake mechanism, anti-repeat feature and single stroke mechanisms were in a safe operating condition. Employees are required to operate the presses daily and are exposed to crush hazards from press malfunctions that would otherwise be found during inspections, including but not limited to allowing the mode selector switch to operate without a key. Maumee Assembly and Stamping, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910. 217(e)(1)(i) which was contained in OSHA inspection number 1223334, citation number 1, item number 4 and was affirmed as a final order on August 2, 2017 with respect to a workplace located at 920 Illinois Ave. Maumee, OH 43537.

29 CFR 1910.217(e)(3): Personal caring for, inspecting and/or maintain power presses, specifically employees performing die setting, were not trained and instructed in the safe method of work before starting work on mechanical power press(es): a.) On or about September 21, 2017, the employer did not ensure employees were adequately trained in the safe operation of the mechanical power presses, including but not limited to the Verson BLK2. Adequate training was not provided regarding the hazards associated with die setting, including mode selection controls, exposing employees to amputation hazards. Maumee Assembly and Stamping, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910. 217(f)(2) which was contained in OSHA inspection number 1223334, citation number 1, item number 5 and was affirmed as a final order on August 2, 2017 with respect to a workplace located at 920 Illinois Ave. Maumee, OH 43537.
