29 CFR 1910.22(a)(2): (2) The floor of each workroom was not maintained in a clean and, to the extent feasible, in a dry condition.  When wet processes were used, drainage were not maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats were not provided:        a) On or about January 23, 2017 and at times prior, an employee was exposed to slipping hazards from spilled machine oil on the floor. The machine oil was coming from CNC machines that were missing cover panels.

The operators were not certified through training and evaluation prior to operating the forklift.

The machine was not provided with a chip guard to prevent flying chips.
