29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about January 18, 2017 at or near 128 Chambord Dr, Maumelle, AR 72113, the employer exposed employees working on the ground or lower edges of the roof to struck-by hazards by not providing them with hardhats to protect their heads from falling or flying objects that could cause physical harm.

On or about January 18, 2017 at or near 128 Chambord Dr, Maumelle, AR 72113, the employer exposed employees operating nail guns to struck-by hazards by not providing them with safety glasses to protect their eyes from flying objects that could cause physical harm their eyes or face.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  On or about January 18, 2017 at or near 128 Chambord Dr, Maumelle, AR 72113, the employer exposed employees installing shingles on a 12/12 pitched roof to fall hazards by allowing them wear full body harnesses and attach lanyards through the front leg straps at the groin.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about January 18, 2017 at or near 128 Chambord Dr, Maumelle, AR 72113, the employer exposed employees to fall from height hazards while mounting and dismounting a ladder that was only extended approximately 1 feet above the upper landing surface to the 12/12 pitched roof they were installing shingles on.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about January 18, 2017 at or near 128 Chambord Dr, Maumelle, AR 72113, the employer exposed employees to fall from height hazards while installing shingles on a 12/12 pitched roof top that was approximately 24 feet above lower levels.  Ricardo Lopez was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13)), which was contained in OSHA inspection number 1183062, citation number 1, item number 1 and was affirmed as a final order on November 16, 2016, with respect to a workplace located at 186 Auriel Circle, Maumelle, AR 72113.
