29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use.     On the west side of the construction site, employees installing stucco material on a residential home worked from a wooden pallet platform elevated by a bobcat skid loader with forks, and were exposed to unprotected fall hazards.  The platform in use was not an approved work platform by the manufacturer for capacity, construction, use, and fall protection.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section.   On the east side of the construction site, employees installing stucco material from a tubular welded frame scaffold without fall protection were exposed to unprotected fall hazards of approximately 18 feet.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.   On the west side of the construction site, the employer did not ensure employees working from a tubular welded frame scaffold system had an attached ladder for safe access and egress, employees climbed the cross bracing and end frames exposing them to fall hazards.
