29 CFR 1910.29(b)(13): When guardrail systems are used around holes that serve as points of access (such as ladderways), the guardrail system opening did not meet the requirements (b)(13)(i) or (b)(13)(ii) of this section:  a)  Blender platform;  On or about January 18, 2017 and at times prior thereto, employees were exposed to fall hazards of 9 feet 3 inches to the concrete below when they were required to access the blender platform to clean out plastic where there was no guarding at the point of access opening.  b)  Color change platform at injection mold # 202;  On or about January 18, 2017 and at times prior thereto, employees were exposed to fall hazards of 7 feet 5 inches to the concrete below when they were required to access a platform to perform a color change where there was no guarding at the point of access opening.  c)  Color change platform at injection mold # 304;  On or about January 18, 2017 and at times prior thereto, employees were exposed to fall hazards of 7 feet 2 inches to the concrete below when they were required to access a platform to perform a color change where there was no guarding at the point of access opening.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:  a)  Production floor;  On or about January 18, 2017 and at times prior thereto, the employer failed to provide employees with the training that would give them the knowledge, skills, and abilities to recognize the hazards associated with equipment, identify the energy sources present, and the methods that should be used to isolate the energy sources on these pieces of equipment.  Employees are required to perform servicing and maintenance on multi-energy sourced equipment (including but not limited to blow molds and injection molding machines).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)  Hanil Deflasher press;  On or about January 18, 2017 and at times prior thereto, the employer exposed employees to amputation hazards when employees were required to load parts into the machine in that the light curtains on the right side of the machine where not operable.  b)  Dongmyung Blowmold #201;  On or about January 18, 2017 and at times prior thereto, the employer exposed employees to crushing hazards when employees were required access the inside of the mold to unjam the conveyor and retrieve fallen product in the bottom of the machine in that the light curtain at the entrance of the machine had been disabled.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment:  a)  Blow mold 203;  On or about January 18, 2017 and at times prior thereto, the employer exposed employees to the hazardous energies present (including electrical, hydraulic, pneumatic, water) on a blow mold machine in that the employer permitted employees to enter the machine clean and perform other servicing and maintenance activities without locking it out in accordance with the energy control procedure.  b)  Injection molding machines including but not limited to 104, 202, 208, and 304;  On or about January 18, 2017 and at times prior thereto, the employer exposed employees to the hazardous energies present (including electrical, hydraulic, pneumatic, water) on injection molding machines in that the employer permitted employees to enter the machines to perform mold changes and lance without locking it out in accordance with the energy control procedure.  The Inzi controls Alabama, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1019.147(d)(2), which was contained in OSHA inspection number 1059088, citation number 1, item number 1a and was affirmed as a final order on September 2, 2015, with respect to a workplace located at 375 Hwy 203, Elba, Alabama 36323.
