29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: JOB SITE: 820 workers were observed on two (2) instances, 1) on top of the main residential structure of a 16:12 pitch roof, actively installing roof shingles, approximately 12' 5" eave to ground level, and 2) on top of the garage of a 8:12 pitch roof, actively installing roof shingles, approximately 8' 9" eave to ground level. The workers were not protected by guardrail systems, safety net system, or personal fall arrest system. The workers were using toeboards/slide guards as the sole means of fall protection. Fitzgerald Roofing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 944694, citation number 1, item number 1, issued on 11/2/2013, and was affirmed as a final order on 11/21/2013, with respect to a workplace located at 1140 S. Rock Hill Road, Webster Groves, MO 63119. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: JOB SITE: 820 Sappington Road, Saint Louis, MO 63126, on or about 1/26/2017, a worker was observed using an aluminum extension ladder to access the roof and the side rails did not extend at least 3 feet above the landing surface. The worker was exposed to a fall hazard.
