29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a)  Production Floor; On or about January 26, 2017 and at times prior thereto the employer exposed employees to potential hazardous energies in that employees were required to work on equipment to include but not limited to Repelletizer(s) which had multiple energy sources such as electric and hydraulic without those employees having been provided with written machine specific lock out procedure(s) to utilize to ensure that those energy sources were controlled while the employees were performing maintenance or servicing on the machine(s).    b)  Production Floor; On or about January 26, 2017 and at times prior thereto the employer exposed employees to potential hazardous energies in that employees were required to work on equipment to include but not limited to Extruders(s) which had multiple energy sources such as electric, water, and air without those employees having been provided with written machine specific lock out procedure(s) to utilize to ensure that those energy sources were controlled while the employees were performing maintenance or servicing on the machine(s).

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment:     a)  Repelletizer #1 (Old); On or about January 26, 2017 and at times prior thereto the employer exposed employees to potential hazardous energies in that employees were required to work on the Repelletizer to replace blades on the cutter without shutting it down and locking it out in accordance with the company's Lockout program established procedures.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):   a)  Repelletizer #1 (Old); On or about January 26, 2017 and at times prior thereto the employer exposed employees to potential hazardous energies in that employees were required to work on the Repelletizer to replace blades on the cutter without isolating the machine from the energy source - employees were using a stop button versus the machines power disconnect (an energy isolating device) to isolate the machine from the energy source.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:    a)  Repelletizer #1 (old); On or about January 25, 2017 and at times prior thereto the employer exposed employees to potential hazardous energies in that employees were allowed to work on the Repelletizer #1 together without all employees applying a personal lockout device on the energy control lockout point(s).    Genpak LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(f)(3)(ii)(D), which was contained in OSHA inspection number 1090051, citation number 1, item number 2 and was affirmed as a final order on October 25, 2016, with respect to a workplace located at 7621 Bill Joseph Parkway, Hope Hull AL.
