29 CFR 1910.334(a)(3)(ii): Attachment plugs and receptacles may not be connected or altered in a manner which would prevent proper continuity of the equipment grounding conductor at the point where plugs are attached to receptacles. Additionally, these devices may not be altered to allow the grounding pole of a plug to be inserted into slots intended for connection to the current-carrying conductors.    1) Jobsite, 3rd floor: A circular saw used by the employees to conduct framing operations was missing the grounding pin thus exposing employees to electrical hazards.    Condition observed on/about 02 FEB 2017.

29 CFR 1926.405(g)(2)(iii): Splices. Flexible cords shall be used only in continuous lengths without splice or tap. Hard service flexible cords No. 12 or larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced.     1) Jobsite, 3rd floor hallway: The extension cord used to power a circular saw was spliced and not in continuous lengths thus exposing employees to electrical hazards. Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    1) Jobsite, 2nd, 3rd floor elevator shaft: An elevator shaft was not protected by a complete guardrail system, or similar fall protection system, thus exposing employees to falls of approximately 25-36 feet.
