On or about February 3, 2017, and at times prior thereto, the framers used a job-made, wooden trash box on the forks of the rough terrain forklift. The box was not secured to the lift with fork pockets and other positive means to prevent tipping. The load ratings for the box are unknown and the manufacturer has not approved of the attachment.
