29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about May 11, 2017, the employer did not conduct periodic inspections of the energy control procedures for the following equipment at least annually:  a. Mori Seiki # 732 Horizontal Mill b. Okuma # 738 Horizontal Mill c. Toyota # 734 Horizontal Mill  Employees were exposed to amputation and laceration hazards.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):     On or about January 27, 2017 employees in the Custom Machine Shop performed servicing and maintenance tasks when performing setups on the following Horizontal Milling Machines:     a. The Mori Seiki #732   b. The Okuma #738   c. The Toyota #734     Employees were not required to control hazardous energy sources in the following sequence:     1. Turn off and shut down the machines.   2. Physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s).   3. Affix lockout or tagout devices to each energy isolating device.   4. Verify that isolation and de-energization of the machines or equipment have been accomplished.     Employees were exposed to amputation and laceration hazards from the unexpected start up and/or the release of hazardous energy.  Alternative protection, such a machine guarding, was not adequate.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1904.19(d).

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not using electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed:    On or about May 10, 2017, maintenance employees who performed voltage testing on energized electrical components (480 volts) did not wear electrical protective equipment appropriate for the level of exposure, to protect against electrical shock and arc flash/blast injuries, including:    1. Hand protection:  Rubber insulating gloves with leather protectors or arc-rated gloves;    2. Apparel: Arc-rated clothing such as arc-rated long-sleeve shirt and pants or arc-rated coverall, and leather footwear;    3. Head and face protection:  Arc-rated face shield, arc-rated hard hat, arc-rated flash suit hood and/or balaclava;    4. Hearing protection:  Hearing protectors (canal inserts).    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
