29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.    a. Pentair Flow Technologies, LLC located in Ashland, OH:  On or about December 4, 2019 the employer did not ensure the written machine specific lockout procedures for the Hunter #1 and Hunter #2 molding machines located in the foundry area, properly identified the electrical lockout point for the equipment.  The procedures stated that the electrical lockout point was the e-stop button which is a control circuit.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees.   a. Pentair Flow Technologies, LLC located in Ashland, OH:  On or about December 4, 2019 the employer did not ensure employees performing pattern changeover operations were properly trained on the company's lockout program.  Employees were not placing a lockout device on the electrical disconnects for the Hunter #1 and the Hunter #2 molding machines located in the foundry area but were rather placing their lockout locks on the e-stops which are considered control circuits.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):(a)   a. Pentair Flow Technologies, LLC located in Ashland, OH:  On or about December 4, 2019 the employer did not ensure the Hunter molding machine #2 was properly locked out prior to employees adjusting the sand molds and bottom plates in front of the mold pusher.  An employee was making an adjustment to the sand mold and bottom plate when the mold pusher unexpectedly advanced, crushing three of the employee's fingers between the mold pusher and the conveyor.  b. Pentair Flow Technologies, LLC located in Ashland, OH:  On or about December 4, 2019 the employer did not ensure employees were properly locking out the Hunter molding machine #2 located in the production area.  Employees were placing lockout devices on the e-stop button located on the machine's control panel instead of placing the lockout device on the electrical disconnect.  c. Pentair Flow Technologies, LLC located in Ashland, OH:  On or about December 4, 2019 the employer did not ensure employees were properly locking out the Hunter molding machine #1 located in the production area.  Employees were placing lockout devices on the e-stop button located on the machine's control panel instead of placing the lockout device on the electrical disconnect.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.   a. Pentair Flow Technologies, LLC located in Ashland, OH:  On or about December 4, 2019 the employer did not ensure the Hunter molding machine #2 was properly guarded to protect employees from pinch point hazards created by the rotation of the drag flask.  b. Pentair Flow Technologies, LLC located in Ashland, OH:  On or about December 4, 2019 the employer did not ensure the Hunter molding machine #2 was properly guarded to protect employees from pinch point hazards created between the pattern lift frame as it passes by the pattern guide wheels.  c. Pentair Flow Technologies, LLC located in Ashland, OH:  On or about December 4, 2019 the employer did not ensure the Hunter molding machine #2 was properly guarded to protect employees from pinch point hazards created by the pattern and the cope flask as the pattern is raised into position.  d. Pentair Flow Technologies, LLC located in Ashland, OH:  On or about December 4, 2019 The employer did not ensure employees were protected from crushing injuries created by the Hunter #2 board pusher and the metal conveyor when they manually activated a mechanical limit switch to retrieve more bottom boards.

29 CFR 1910.1030(f)(1)(i): The employer did not make available the hepatitis B vaccine and vaccination series to all employees who have occupational exposure, and post-exposure evaluation and follow-up to all employees who have had an exposure incident.  a. Pentair Flow Technologies, LLC located in Ashland, OH:  On or about December 4, 2019 the employer did not ensure designated first aid responders were provided with the Hepatitis B vaccination series prior to responding to a first aid incident involving potential bloodborne exposures.  Employees provided first aid services and cleaned blood from equipment and surfaces during the response.
