29 CFR 1910.95(g)(8)(ii)(B): Unless a physician determines that the standard threshold shift is not work-related or aggravated by occupational noise exposure, the employer did not ensure, when a standard threshold shift occurs, that for employees already using hearing protectors, employees be refitted and retrained in the use of hearing protectors and provided with hearing protectors offering greater attenuation if necessary: a.) All American Acquisitions, LLC. dba All American Hose, Weaving Department, on or about February 8th, 2017, and times prior thereto: An employee working in the weaving department area of the facility experienced a standard threshold shift in hearing, the employer did not conduct refitting and retraining on hearing protection with this employee. Additionally, the employer did not determine if the employee required hearing protection with greater attenuation.

All American Acquisitions, LLC. dba All American Hose, Weaving Department, on or about April 5, 2017, and times prior thereto: Employees were modifying hearing protection onsite in an attempt to ensure a proper fit. The employer did not ensure employees were wearing hearing protection correctly during work operations in the weaving department.

29 CFR 1904.10(a): If an employee's hearing test (audiogram) reveals that the employee has experienced a work-related Standard Threshold Shift (STS) in hearing in one or both ears, and the employee's total hearing level is 25 decibels (dB) or more above audiometric zero (averaged at 2000, 3000, and 4000 Hz) in the same ear(s) as the STS, you must record the case on the OSHA 300 Log: a.) All American Acquisitions, LLC. dba All American Hose, on or about February 8th, 2017 and times prior thereto: In 2016, one recordable case of hearing loss was not recorded on the Company's 300 Logs.
