29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) were not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Loom Loft, on or about October 25, 2017, employees performing servicing and maintenance, including, but not limited to removing lint from the loom, did not follow the energy control procedures before having hands inside the loom, near moving parts.

29 CFR 1910.219(f)(1): Gears were not guarded in accordance with one of the methods specified in 29 CFR 1910.219(f)(1)(ii) and (f)(1)(iii): a) Loom Loft, on or about October 25, 2017, and times prior thereto, the rotating gears under the loom created a nip point. The gears were not guarded to prevent employees from having their hands or fingers near the moving parts. On October 20, 2017, an employee was injured when he was caught by the rotating gears of Loom #28.
