29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    On or about February 4, 2017, the employer did not ensure that employees unjamming and cleaning Machine No. 8, Crabtree 1988/1989, model Fast Ready, S# F5948, were protected from moving and rotating components by performing the following sequence of actions prior to servicing the machine:      1) Machine or equipment shutdown   2) Machine or equipment isolation by physically operating isolating devices  3) Affix lock(s) and tagging devices to energy isolating device(s)   4) Verification of effective isolation.     Employees were exposed to degloving and amputation hazards.     J. L. Clark, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d), which was contained in OSHA inspection number 1052892, citation number 1, item number 1(b) and was affirmed as a final order on August 24, 2015, with respect to a workplace located at 923 23rd Avenue, Rockford, IL 61104.       Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1910.19(d).
