29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: Fabrication department, Littel coil line: on or about March 16, 2017, operators assigned to perform regular setup tasks (servicing) on energized equipment in the point-of-operation and associated danger zones were not provided with developed and documented machine-specific hazardous energy control (lockout tagout) procedures to ensure safety in accordance with the standard. Operators were exposed to unexpected machine start-up likely to result in serious injuries due to caught in-between hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)

29 CFR 1910.147(c)(7)(iii)(A): Retraining shall be provided for all authorized and affected employees whenever there is a change in their job assignments, a change in machines, equipment or processes that present a new hazard, or when there is a change in the energy control procedures. Fabrication department, Littel coil line: on or about March 16, 2017, the employer did not provide hazardous energy control lockout/tagout authorized employee retraining for a new coil line operator that was previously trained as the affected employee in the position of an oven tender. An operator was assigned to perform regular setup tasks independently and had his hands exposed to new equipment caught-in hazards from rotating rollers ingoing nip point likely to result in serious injury during servicing activities. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: Fabrication department, Littel coil line - on or about March 16, 2017, employees performing setup tasks (servicing) in the point-of-operation and associated danger zones were not required to apply energy controls in the following sequence prior to starting work: (a) prepare for shut down by recognizing energy hazards and its controls, (b) turn off and shut down the machines, (c) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (d) affix lockout or tagout devices to each energy isolating device, (e) render safe all potentially hazardous stored or residual energy, (f) verify that isolation and de-energization of the machine or equipment have been accomplished. Operators assigned to perform setup (servicing) on energized equipment were exposed to machine caught-in hazards likely to result in serious injury because hazardous energy controls (lockout) were not implemented. Alternative measure, such as, but not limited to relying on a stopped cycle and/or program to prevent unexpected start-up on energized machines did not ensure equivalent safety. Other safeguards were not provided to prevent operators' access and/or disable hazardous operations start-up prior to exposure. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d). The J. L. Clark, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d), which was contained in OSHA inspection number 1052892, citation number 01, item number 001B and was affirmed as a final order on August 24, 2015, with respect to a workplace located at 923 23rd Ave., Rockford, IL 61104.
