29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6)  On or about January 30, 2021 an employee was exposed to amputations while clearing jams from the hemmer machine on the Murray body maker line. The employer did not ensure that the employee was protected from amputation hazards using lockout tagout procedures that would require the following:  1) Machine or equipment shutdown  2) Machine or equipment isolation  3) Lockout or tagout device application  4) Verification of isolation.   J.L. Clark , LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(d), which was contained in OSHA inspection number 1249482, citation number 2, item number 1 and was affirmed as a final order on January 10, 2018, with respect to a workplace located at 923 23rd Ave, Rockford, IL 61104.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
