29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.    On or about February 6, 2017, Employees performing masonry installation on the chimney of a residential property were exposed to a fall hazard of 23.9 feet when the employer failed to ensure a ladder was available for access to the working levels.      Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.  Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part.    On or about February 6, 2017, employees performing masonry installation at a residential property were exposed to a 23.9 foot fall and the employer failed to ensure adequate fall protection was provided.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail.    On or about February 6, 2017, employees performing masonry installation from a scaffolding on a residential property were exposed to a 23.9 foot fall when the employer failed to ensure the working levels of the scaffold were fully planked.     The employer was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number 741801, citation number 1, item number 1 and was affirmed as a final order on December 20, 2012, with respect to a workplace located at 348 S. Waiola Ave., La Grange, Illinois.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
