29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  a) 110 South University (Mobile, Alabama); On or about the February 15, 2017, and at times prior thereto, the employer exposed his employees to a 14 foot fall hazard in that employees performing masonry operations were allowed to work and/or walk on the northeast scaffold system where the platforms were not fully planked or decked on the second buck of the scaffolding system.  b) 110 South University (Mobile, Alabama); On or about the February 15, 2017, and at times prior thereto, the employer exposed his employees to a 14 foot fall hazard in that employees performing masonry operations were allowed to work from the northwest scaffold system where the platforms were not fully planked or decked on the second buck of the scaffolding system.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a) 110 South University (Mobile, Alabama); On or about the February 15, 2017, and at times prior thereto, the employer exposed his employees to a 14 foot fall hazard in that employees were allowed to perform masonry work from the top level of a two buck scaffold without fall protection such as but not limited to guardrail systems, personal fall arrest systems, or safety net systems. The scaffold was located on the northeast face of the structure.  b) 110 South University (Mobile, Alabama); On or about the February 15, 2017, and at times prior thereto, the employer exposed his employees to a 14 foot fall hazard in that employees were allowed to perform masonry work from the top level of a two buck scaffold without fall protection such as but not limited to guardrail systems, personal fall arrest systems, or safety net systems. The scaffold was located on the northwest face of the structure.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:  a) 110 South University (Mobile, Alabama); On or about the February 15, 2017, and at times prior thereto, the employer exposed his employees to a 14 feet fall hazard in that employees performing masonry work were allowed to climb up the sides supports of the northeast scaffold system to access the second buck without the use of a ladder or other means of safe access.  b) 110 South University (Mobile, Alabama); On or about the February 15, 2017, and at times prior thereto, the employer exposed his employees to a 14 feet fall hazard in that employees performing masonry work were allowed to climb up the sides supports of the northwest scaffold system to access the second buck without the use of a ladder or other means of safe access.
