29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice: General Aviation Facility ramp- (a) The nails holding the ramp planks down are coming up, making the walking surface unstable, exposing employees to injuries. (b) The planks are rotting and do not provide walking surface with traction.

Handrails and stair rail systems are smooth-surfaced to protect employees from injury, such as punctures or lacerations, and to prevent catching or snagging of clothing; General Aviation Facility ramp- (a) The 2 X 4 handrail leading up the ramp had nail heads protruding upward from the top of the board, exposing employees to injury.

29 CFR 1910.304(a)(2): The grounded conductor was attached to a terminal or lead that reversed the designated polarity. General Aviation Facility modular unit- (a) Two 110 volt receptacles were tested with the IDEAL Sure-Test Analyzer and determined to have a reversed polarity condition, exposing employees to electrical shock hazards.
