29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: On or about 02/22/2017, employees working at Mitek USA, Inc., located at 301 Fountain Lakes Industrial Drive, St. Charles, MO, were operating forklifts without having completed the required refresher training. There were approximately 14 forklift operators that had not received evaluation/training since 02/21/2012 to 11/21/2013.

Portion(s) of the Hyster forklifts plastic seat cover and the foam was missing exposing the operator to a wooden frame. The Hyster forklift with the defective operators seat had a Serial Number of: A108D 1700T.
