29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a) On or about January 18, 2017 and at times prior thereto Barry Hayes Construction, LLC did not ensure that its sub-contractor protected its employees from exposure to fall hazards greater than 15 feet through a roof access floor hole while installing a rubber membrane roof, where no fall protection was used. NOTE-Abatement certification and documentation are required for this item. Barry Hayes Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.502(b)(13), which was contained in OSHA inspection number 1151755, citation number 1, item 1a, issued on June 17, 2016 and was affirmed as a final order on July 14, 2016, with respect to a workplace located at 521 Carrie Rex Avenue, Melba, ID.
