29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a) Amneal Pharmaceuticals, LLC/Piscataway, NJ- Laboratory Area/Hood Room: Eyewash facilities were not provided within the work area for immediate emergency use where employees transfer and mix acids such as Hydrochloric Acid, Sulfuric Acid, and Formic acid, and bases such as Ammonium hydroxide and Sodium Hydroxide.  Condition observed on or about February 16, 2017.

29 CFR 1910.134(k)(1)(iv): The employer did not ensure that each user could demonstrate knowledge of how to inspect, put on, and check the seals of the respirator:  a) Amneal Pharmaceuticals, LLC/1 New England Avenue, Piscataway, NJ- Throughout the Laboratory Area: Employees could not demonstrate the knowledge of how to inspect, put on, and check the seals of the respirator they were required to wear, such as but not limited to elastomeric air-purifying half- face respirator (North) and filtering face piece respirator (3M). Employees handling and mixing chemicals are exposed to airborne particulates and chemical vapors.  Condition observed on or about February 16, 2017.

29 CFR 1910.134(h)(1)(ii): The employer did not ensure that respirators issued to more than one employee were cleaned and disinfected before being worn by different individuals:  a) Amneal Pharmaceuticals, LLC/1 New England Avenue, Piscataway, NJ- QC Laboratory Area: An elastomeric air-purifying respirator North 7700 series with cartridges was shared between employees working within the laboratory area, without prior cleaning or disinfecting the respirator.  Condition observed on or about February 16, 2017.

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve:  a) Amneal Pharmaceuticals, LLC/1 New England Avenue, Piscataway, NJ- QC Laboratory Area: The employer did not ensure that respirators used in the laboratory area are properly stored. An elastomeric air-purifying respirator, North 7700 series, was stored on the top of acid cabinet exposing the respirator to contamination from dust and corrosive chemicals.  Condition observed on or about February 16, 2017.
