29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   A) Front of building, on or about February 28, 2017, employees were installing shingles onto the roof of a residential building without the use of fall protection, exposing the employee to fall hazard of 10 to 16 feet.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:   A) Front of house, on or about February 28, 2017, employees used a TranzSporter Hoist to access the roof of a home, exposing the employees to a fall hazard.
