Road, Findlay, Ohio, 45840: On or about October 9, 2024, the employer failed to ensure that each affected employee used appropriate eye or face protection when performing tear-off roofing activities atop a commercial structure while exposed to potential struck by hazards (flying particles/debris) from equipment such as, but not limited to, powered blowing tools.

Road, Findlay, Ohio, 45840: On or about October 9, 2024, the employer failed to ensure employees were protected from fall hazards while working atop a 4:12 sloped roof of a commercial structure.  Employee(s) performing roofing operation activities in this condition were exposed to fall hazards of approximately 12 feet to the ground below.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee who might be exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Simon Trejo at worksite located at 8210 Co. Road, Findlay, Ohio, 45840: On or about October 9, 2024, the employer failed to ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternate measures while performing roofing operations from elevated surfaces greater than six feet from the ground below.
