29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  (a) At a residential construction site located at 624 E. 96th Street in Kansas City, MO 64131: Employees were exposed to potentially serious injuries and/or illnesses in that the employer had not developed, or successfully implemented, an effective safety and health program. On 2/22/2017 at the residential construction site 5 employees were exposed to fall hazards while working from the roof and they had not been trained in the regulations applicable to their work.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a). At the residential construction site located at 624 E 96th Street, Kansas City, Missouri 64131- Employees were exposed to overhead hazards from employees and materials overhead and were not protected from impact  by the use of hard hats.

29 CFR 1926.102(a)(1): The employer should ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a) At the residential construction site located at 624. E 96th Street, Kansas City, MO 64131 - Employees were observed working without eye protection while using pneumatic nail gun and hammers without the use of many means of eye protection.

29 CFR 1926.152(a)(1): Containers other than approved metal safety cans or DOT approved containers were used for the handling and use of flammable liquids in quantities of 5 gallons or less:  (a) At the residential construction site located at 624 E 96th Street, Kansas City, M) 64131. The employer was using a gasoline canister for the gasoline powered air compressor, and the can was not an approved metal safety can or DOT approved container.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) At the residential construction site located at 624 E 96th Street, Kansas City, Mo 64131:  Employees were exposed to fall hazards while working on a roof without the use of fall protection. The approximate distance from the eave of the house to the ground was  21.3 feet on the east side, 16.1 feet on the west side, and 6.5 feet on the north side.
