29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees: On or about March 7 , 2017, an Airset employee did not affix an energy isolating device to the electrical disconnect for the IMF Continuous Mixer (M# T5618, S# BR272-858) Airset sand/resin mixer. The Airset employee had closed a lockout hasp on the Continuous Mixer's electrical disconnect lock receptacle and had placed the shank of his lockout padlock through the hasp. However, the lock's shank was not secured into the locking mechanism of the padlock and the key was left in the lock as the employee conducted work on the machine.
