29 CFR 1910.27(a): Scaffolds used in general industry did not meet the requirements in 29 CFR part 1926, subpart L (Scaffolds): 29 CFR 1926.453(a)(2): Field modification of the aerial lift was not certified in writing by the manufacturer or by any other equivalent entity, such as a nationally recognized testing laboratory, to be in conformity with all applicable provisions of ANSI: a) Cherry Picker - Employees using the Hydraulic Elevator were exposed to fall hazards from field modifications that were not certified by the manufacturer or any other equivalent entity, on or about February 28th, 2017. No abatement certification or documentation required.

29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and de-energization of the machine or equipment had been accomplished: a) KW Monorail Switch - An employee working on a monorail switch was exposed to stored energy from a repaired part in the wrong configuration, on or about February 23rd, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
