29 CFR  1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a. Hydro line - On or about April 25, 2022, employees were exposed to a fall hazard to a lower level at the hydro pit which had one side not protected by the use of a fall protection system.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1910.110(f)(2)(i):Liquefied petroleum gas container(s) in storage were not located so as to minimize exposure to excessive temperature rise, physical damage, or tampering from unauthorized persons:  a. LPG storage - On or about April 25, 2022, employees were exposed to a fire/explosion hazard from unsecured, storage of 31 LGP tanks. The tanks were kept in an open building with no doors or method of preventing tampering by unauthorized persons.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and moving parts.  a. Maintenance Shop - On or about April 25, 2022, employees were exposed to a caught in hazard from an unguarded vertical band saw blade, their was 2 inches of unguarded blade under the table rest and the back side of the upper guard was not enclosed exposing employees to 19 inches of unguarded blade.  b. Hydro-line - On or about April 25, 2022, employees were exposed to a caught in hazard from unguarded chain conveyor used to move the moldings.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1910.28(b)(3)(iv):  The employer did not provide that each employee be protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used.  a. Drum Crusher - On or about April 25, 2022, the fixed ladder did not have a self closing gate at the top at the entrance to the hole, employees were exposed to a fall hazard.  b. Silo - On or about April 25, 2022, the fixed ladder did not have a self closing gate at the top at the entrance to the hole, employees were exposed to a fall hazard.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1910.179(h)(4):  Hooks did not meet the manufacturer's recommendations:  a. Hydro-line - On or about April 25, 2022, the hook on the gantry crane wire rope used to raise and lower the molds had a hook installed with a safety latch, however the safety latch was not maintained as manufacturer designed as it had slipped outside of the locking area rendering the latch ineffective, exposing employees to a struck-by hazard.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
