29 CFR 1910.212(a)(3)(ii) as required by 29 CFR 1960.8(b):  The point of operation of machines whose operation exposes an employee to injury, were not guarded:  The blade guard lock was missing from the Bradley Paper Cutter.   The Dept of Agriculture-FSA was previously cited for a violation of this occupational safety and health standard or similar standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1167409 Notice number 1, Item 4 and was affirmed as a final order on 9/28/2016 with respect to a workplace located at 1513 N. Ankeny, Blvd. Suite 1. Ankeny, IA 50021.                                          Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos, which you believe will be helpful.  The abatement certification sheet is enclosed with the notice(s).
