29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and Unit 2 Staging Area, Press B in 121 building, 121 Press Department - On or About February 24, 2017, the employer did not guard a pinch point between a moving hook and stationary parts of the cylinder and the printing press. An employee sustained a left index fingertip amputation while hoisting a metal printing press cylinder onto a printing press.
