29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. a) Extruder Department at Extruder Machine #1: On or about December 14, 2016 and at time prior, the employer exposed employees to amputation hazards in that Specific Procedural Steps were not developed for clearing jams on the extruder machine.

29 CFR 1910.212(a)(3)(iii): Special handtools for placing and removing material shall be such as to permit easy handling of material without the operator placing a hand in the danger zone. Such tools shall not be in lieu of other guarding required by this section, but can only be used to supplement protection provided. a) Extruder Department at Extruder Machine #1: On or about December 14, 2016, and at time prior, the employer exposed employees to amputation hazards in that employees had to place hands in proximity to a pinch point when guiding the hopper while it is being lowered down onto the kneading chamber.
