29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed. a. Production area - On or about August 28, 2019, authorized employees were required to perform maintenance and/or service on various machinery including, but not limited to Peel and Press machines. The employer did not ensure that periodic inspections of the energy control procedures were performed and certified at least annually.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a. On or about August 22, 2019, employees were exposed to machine hazards associated with moving parts during servicing un-jamming and cleaning the Peel and Press line #4, The employer failed to implement energy control application steps as lockout/tagout devices were not applied to the electrical disconnect {per the 1910.147(d)(4) requirements}. As a result, the remaining applicable energy control elements, including dissipation of residual energy [energy [(d)(5)(1) and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.
