29 CFR 1910.147(c)(4)(i):  Procedures were not developed, documented, and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    At 3125 Boschertown Road, St. Charles, MO, specifically inside of the Fabrication Department, specific lockout/tagout procedures were not developed in writing by the employer or utilized by the authorized employees.  This violation was observed on 03/21/17. The following machinery were identified:    a/  Accurpress Press Brake, m/n 7608, s/n 10289.    b/  Accurpress Press Brake, m/n 71008, s/n 8093.    c/  Accurpress Press Brake, m/n 710010, s/n 11800.    d/  Wysong Press Brake, m/n PHP100-120, s/n HPB46-123X.    e/  Amada Press Brake, m/n 3512LD, s/n 35120063.    f/  Walsh Mechanical Punch Press, m/n 65-X, s/n 9506.    g/  Horn CNC Tool Pipe Bender, m/n 2LCNC.    h/  Pines 5T Bender, m/n 5T, s/n 44355-64064.    i.  Atlantic Shear, m/n HDE 10X1/4, s/n 91103703.    At 3125 Boschertown Road, St. Charles, MO, specifically inside of the Galvanizing Department, specific lockout/tagout procedures were not developed in writing by the employer or utilized by the authorized employees.  This violation was observed on 03/21/17. 29 CFR 1910.147(c)(7)(i)(A): Each authorized employee did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:    At 3125 Boschertown Road, St. Charles, MO, specifically inside of the Fabrication Department, the employer did not ensure that suitable instruction was provided to the authorized employees in the recognition of applicable energy sources, i.e., electricity, hydraulics, gravity, the type and magnitude of each energy source, i.e., p.s.i. and volts, and the methods and means necessary for energy isolation and control, i.e. location of the energy isolation device and the type of lockout/tagout device to be applied.  This violation was observed on 03/21/17.

This violation was observed on 03/21/17.     National Cart Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1069767, citation number 1, item number 1, and was affirmed as a final order on 07/16/15, with respect to a workplace located at 3125 Boschertown Road, St. Charles, MO 63301.
