29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    On or about 08/08/2019, National Cart Co., located at 3125 Boschertown Road, St. Charles, MO, did not conduct timely periodic inspection(s) in accordance with the standard.  Three of the four authorized maintenance personnel were out of date.  Their last dated 07/27/2018.  The employer had various equipment with more than one energy source:    The Torq-Pac Notcher with pneumatics, electrics and potential energies;  The Amanda Turret Punch with electrics and pneumatics; and   The Scotchman Cold Saw with electrics and pneumatics.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about 08/08/2019, National Cart Co., located at 3125 Boschertown Road, St. Charles, MO, the victim was exposed to hazards related to the Torq-Pac Notcher.  The guarding to keep body parts was not properly adjusted.  The training manual did not provide the required adjustment height for the ingoing and outgoing guards.    The National Cart Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1334350, citation number 1, item number 1 and was affirmed as a final order on 08/28/2018, with respect to a workplace located at 3125 Boshertown Road, St. Charles, MO 63301.

29 CFR 1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):   On or about 08/08/2019, National Cart Co., located at 3125 Boschertown Road, St. Charles, MO, had not established a Hazard Communication Program complete with all the required corresponding Safety Data Sheets (SDS).  The employer did have Material Safety Data Sheets (MSDS) for various products/materials.  The employer had not converted everything to Safety Data Sheets.  There were such products as:  1)     Buckeye Gone H2O2;    2)     Buckeye Green Apple; and    3)     EA-744 Burnishing Compound.
