(a) At the BLM Southern Nevada District Hallway across GIS 191, the energized wall outlet used to plug-in electrical equipment had a lose and cracked cover exposing employees to electrical shock.

29 CFR 1910.36(g)(2): Exit access(es) were not at least 28 inches (71.1 cm) wide at all points. a) At the BLM Southern Nevada District, Geologist's office, desk and furniture were obstructing the exit access, thereby reducing it to 18 inches wide. Employees had to maneuver around the narrowed exit access thereby impeding egress. b) At the maintenance crew's room, desk and furniture were obstructing the exit access, thereby reducing it to 21 inches wide. c) At the Secretary's office, counter furniture and trash container was obstructing the exit access, thereby reducing it to 27 inches wide. d) At the BLM Southern Nevada District Manager's office, desk and file cabinet were obstructing the exit access, thereby reducing it to 24 inches wide. The Bureau of Land Management was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.36(g)(2), which was contained in OSHA inspection number 1085493, citation number 2, item number 1, and was affirmed as a final order on 10/12/2015, with respect to a workplace located at BLM, 2610 Sweetwater Ave., Lake Havasu, AZ 86406.
