29 CFR 1910.178(l)(6): The certification did not include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation: a) Storage area: On or about May 3, 2023, certification containing the date of the evaluation and the name of the person who conducted employees powered industrial vehicle training at the facility was not completed as required. Employees authorized to operate the company's Doosan 25 forklift and others working in the areas where fork truck operations occurred were potentially exposed to struck-by and/or crushing hazards.
