29 CFR 1910.212(a)(2): General requirements for machine guards. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself. (a) On or about March 21, 2017 in the Bowl Forming Department, Peerless Machine & Tool Corp #8562 (Model: M40T, S/N: 5-07) by the cutter access door the cutter would complete the cycle after the door was opened before shutting down exposing employees to a caught in and struck by hazard.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the sequence outlined in 29 CFR 1910.147(d)(1)-(6): (a) On or about January 4, 2017 and thereafter in the Roll Tray Department, Roll Tray Machine (Asset #8615) energy control procedures were not conducted prior to performing maintenance and/or servicing such as but not limited to roll changes. Employee(s) performing these tasks in an energized condition were exposed to caught in and struck by hazards at the machine's point of operation on the cutting knives. Alternative protection, such as machine guarding, was not adequate. (b) On or about January 9, 2017 in the Roll Tray Department, Roll Tray Machine (Asset #8624) energy control procedures were not conducted prior to performing maintenance and/or servicing such as but not limited to adjusting the paper in the machine. Employee(s) performing these tasks in an energized condition were exposed to caught in and struck by hazards at the machine's point of operation on the cutting knives. Alternative protection, such as machine guarding, was not adequate. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)], were not implemented to protect employees from machine hazards such as, but not limited to caught in and struck by. The Graphic Packaging International, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1910.147(d)(2)), which was contained in OSHA inspection number 1033327, citation number 1, item number 1b and was affirmed as a final order on August 1, 2016, with respect to a workplace located at 1171 W. Center Street, Marion, Ohio. The Graphic Packaging International, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1130239, citation number 1, item number 1 and was affirmed as a final order on August 4, 2016, with respect to a workplace located at 400 East North Avenue, Carol Stream, Illinois.
