29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a.) On or about March 21, 2017, setup employees were exposed to crushing and struck by injuries in that the #3 Walking Beam was not locked out prior to clearing off a rubber pad inside the V-max 4.8 machine. The employer did not ensure energy control procedures were documented and utilized to control the pneumatic energy before cleaning off the rubber pad.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a.) On or about March 21, 2017, the employer did not ensure employees were trained in the purpose and function of the energy control program. Employees were required to clean off the black rubber pad inside the V-Max 4.8 machine #3 walking beam and were not adequately trained in proper lock out procedures exposing them to crushing hazards from the extending air tester.
