SB Specialty Metals LLC; Shipping/Receiving department - On or about 3/23/17, and at times prior thereto, a stairway that lead to an overhead storage area did not have a standard mid-rail installed. Employees were exposed to a fall height of approximately 14 feet.

SB Specialty Metals LLC; HYDM band saw - On or about 3/23/17, and at times prior thereto, the fan on the motor to saw #18 (HDYM horizontal band saw) was exposed. The casing around the motors fan had large holes in it which caused several areas of the fan to be exposed.
