29 CFR 1910.178(a)(4):Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a.) SB Specialty Metals LLC - On or about 9/21/22, and at times prior thereto, a jib boom forklift attachment was used on the Clark forklift (serial #685-0284-150) to move metal rolls. The company had not obtained the forklift manufacturer approval prior to using the attachment. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a.) SB Specialty Metals, LLC - On or about 9/21/22, and at times prior thereto, a jib boom front end attachment was used on the Clark forklift (serial #685-0284-150) to move solid metal rolls. The jib boom attachment was not noted on the Clark Forklift's load/data plate ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
