29 CFR 1917.24(a): Employees were not removed from an enclosed space when the carbon monoxide concentration exceeded a ceiling of 100 parts per million: (a) A mechanic who operated service truck #639 was exposed to carbon monoxide at a concentration of 108 parts per million (ppm) after entering the enclosed bed of the truck to turn off a gasoline powered air compressor used to operate pneumatic tools. ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information: (Marine Terminals Reference: 1917.28) (a) As of 3/23/17, a mechanic in the chassis yard and a mechanic working in the main shop were potentially exposed to hazardous chemicals (e.g., welding rods, spray paint, brake fluid, degreasers, solvents, grease, fuels, acetylene/oxygen) and had not been trained on the new chemical container label elements and the new format and types of information on each section of safety data sheets (SDSs) with respect to revisions to the standard in accordance with the Globally Harmonized System of Classification and Labeling of Chemicals (GHS).
