29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Production area; Energy isolation devices were not used during clearing jams, blade changes, and cleaning operations on equipment such as, but not limited to powered rollers and slitters. General Plastic Extrusions, Inc. was previously cited for a serious violation of the Occupational Safety and Health standard 29 CFR 1910.147(d)(4)(i) which was contained in OSHA inspection #992169 citation 1, item number 2 and affirmed as final order on March 6, 2015 with respect to the workplace at 1238 Kasson Dr., Prescott, WI 54021.
