29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): a) Bakery floor - Procedures were not implemented for the "stamping machine" on clearing of jams and specific steps for energy isolation, exposing employees to getting caught in or caught by moving machine parts, on or about March 25, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.212(a)(3)(i): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefore, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a) Bakery floor: On or about March 25, 2017, an employee sustained a serious finger injury from contact with moving machine parts. No guard was provided to prevent contact with the dough stamping process of the "Cuttet" machine. No abatement certification required for this item.
