29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section: (a) MAPLE DONUTS FACILITY - The employer had not developed machine specific lock out tag out procedures for plant equipment, including the Line Two Stamper machine, on or about September 5, 2018. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement. Maple Donuts Incorporated was previously cited for a violation of this occupational safety and health standard, 29 CFR 1910.147(c)(4)(ii), which was contained in OSHA inspection number 1222839, citation number 1, item number 1, and was affirmed as a final order on June 30, 2017, with respect to a workplace located at 3455 East Market Street, York, PA 17402.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: (a) PRODUCTION FLOOR, BELOW BREAKER PANEL NINE - The area below the elevated breaker panel (breaker panel nine) was used to store a pressure washer, on or about September 5, 2018. No abatement certification or documentation required.
