29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about April 5, 2017, at the above addressed job site, an employee was not protected from an approximate 10-foot fall hazard while installing a drive thru canopy.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   On or about April 5, 2017, at the above addressed job site, an employee was exposed to a 10-foot fall hazard when using an A-frame ladder which did not extend at least 3 feet above the upper landing surface while gaining access to a roof of a canopy.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  On or about April 5, 2017, at the above addressed job site, an employee was exposed to an approximate 10-foot fall hazard when using an A-frame ladder as a straight ladder when gaining access to an overhead working surface.
