Consulting, Inc., and EMK Consultants, Inc. were safe from health and safety hazards by conducting frequent and regular job site inspections by a competent person. Employees of subcontractors were exposed to excavation-related hazards, such as, but not limited to, cave-in hazards while working in an unprotected excavation deeper than 5 feet.             Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
